Patient presented in poor mental, physical, emotional, and spiritual health. Reported progressive decline in quality of life with mounting hopelessness and difficulty sustaining the desire to live. Uncontrollable substance use included opioids, alcohol, stimulants, and cigarettes. Experienced chronic back problems.
